Multiple sclerosis is a chronic, often d!sab!ing disease that attacks a person's nervous system. The progress, severity and specif- ic symptoms of the disease can't be predicted. Symptoms may range from numbness to paralysis and blind- ness. Most people with MS are diagnosed between the ages of 20 and 40, but the unpredictable physical and emotional effects can last for the rest of their lives. There are more than 1,000 Delawareans who live with MS. For more information about the Celebration of Achievement, con- tact Elizabeth Carter at (302) 655- 5610. What should I know about hematuria? By Vincent Killeen, M.D., F.A.C.O.G. Hematuria is a term used to describe blood in the urine. It can be microscopic, needing a micro- scope for diagnosis, and macro- scopic, when the aid of a micro- scope is not needed. Based on information contained in recent journal articles, about 15 to 20 percent of people excrete some red blood cells in their urine, so defining significant hematurial is of clinical significance and importance. Remember that many things can influence the initial diagnosis: inadequate storage and collection recent heavy exercise other origins, such as uterus, vagina, or vulva drugs or foods with dye technique The correlation of hematuria to signs and symptoms is of clinical importance in making the assess- ment of hematuria, doctors must take a thorough history and per- form a physical exam, an endoscopy of the urethra and blad- der, and get X-rays of the kidneys and upper and lower urinary tracts (IVP). As I said, 15 to 20 percent of hematuria are found not to have a recognizable cause and are ulti- mately labeled as idiopathic or essential. However, it should also KILLEEN be stated that if you follow these with essential hematuria for a two year period, 20 percent are likely to show a detectable lesion. Trauma can lead to hematuria, sometimes requiring surgical intervention. Strenuous exercise may induce bladder trauma result- ing in hematuria. Hematuria may also be due to kidney stones, and is usually asso- ciated with renal colic, kidney pain. Nephritis (inflammation of the kidney) is usually asso- ciated with hyperten- sion, edema, renal insuf- ficiency or failure and electrolyte changes. Drugs and radiation can also lead to hematuria. Rectal or lower intestinal lesions may be mistaken causes for hematuria. Endometriosis of the bladder usually presents with cyclic hematuria and associated pelvic symptoms such as dysmen- norrhea, pelvic pain, infertility or adnexal pathology. Blood dycrasias such as sickle cell needs to be ruled out. Anti-coagulation and non- steroidal inflammatory drugs (NSAIDs) may independently produce hematuria. The presence of hematuria may indicate a serious lesion. Its detec- tion and preliminary evaluation is simple and usually carries a favor- able risk-benefit ratio.
